I, The undersigned parent(s), Hereby consent to my child, participating in the Vacation Bible School connected with Heritage Baptist Church of Palmyra, New York on July 23-25. I certify that my child is able to participate in any and all activities. If my child has medical conditions which may relevant to a physician in the event of an emergency, I have listed them below. If I cannot be reached within a reasonable amount of time, as determined by church officials, I hereby authorize his/her Heritage Baptist Church leader to make emergency medical decisions for my child. If there are activities that I do not want my child to be involved in, I have listed them below.
I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL RISKS WHICH MAY BE ENCOUNTERED DURING SAID ACTIVITIES, INCLUDING ACTIVITIES PRELIMINARY AND SUBSEQUENT THERETO. I do, for myself and my child, heirs and assigned, irrevocably and unconditionally release, acquit, and forever discharge Heritage Baptist Church and its agents, employees, and volunteers from any and all liability actions cause, claims, expenses, obligations, and my child’s participating in Vacation Bible School or any other related activities including, but not limited to, any injury to my child, property, even injury resulting in death.
I expressly agree that this release, waiver, and indemnity are intended to be broad and inclusive as permitted by New York State. If any portion hereto is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto.
I further state that I HAVE CAREFULLY READ AND UNDERSTAND THE FOREGOING RELEASE AND KNOW THE CONTENTS HEREOF AND I SIGN THIS RELEASE AS MY OWN FREE ACT. I understand that this is a legally binding agreement.
If Applicable:
I hereby agree that my child may be transported to and from Heritage Baptist Church by the holder of this consent and release form or by a member of Heritage Baptist Church.
By signing below I give Heritage Baptist Church permission to photograph my child for church purposes. I understand these photos may be used for various activities, events, recognition, promotional material and church history records. I further acknowledge that these activities may be live-streamed on YouTube for the benefit for children viewing remotely and consent to the same.
